Application Process

A Medical Leave of Absence (MLOA) is an option available to those Northeastern students who develop a major medical condition that precludes class attendance, completion of requirements, and/or co-op.

Medical leave requests must be initiated through University Health and Counseling Services by submitting a completed request form and documentation from a clinician supporting the medical leave.

Supporting documentation must include: diagnosis, treatment plan, medications (if applicable), and the reason the medical leave is recommended.

Eligible for Medical Leave of Absence

  • Have a medical or mental health concern that has impaired academic progress
  • Apply before the deadline
  • Has clinical documentation that meets the standards of MLOA
  • Submits a completed MLOA Request form
  • Completes the process in a timely manner

Ineligible for Medical Leave of Absence

  • Participation in a final assignment/exam
  • Student is currently or will be academically dismissed for the term they are seeking a Medical Leave. Medical Leave is not available to prevent or remove an academic dismissal
  • There is no clinical reason or support; or it does not meet the standards
  • Another type of leave is more appropriate
  • Application is received after the deadline

Requirements while on Medical Leave of Absence

  • Participation in treatment for the duration of the Medical Leave of Absence
  • Treatment must be to address the condition(s) for which the MLOA was requested
  • Follow the clinical recommendations of providers

Re-entry from Medical Leave of Absence

Re-entry from a medical leave requires receipt of all documentation delivered to UHCS on or around one month prior to the planned re-entry to classes. Requests for re-entry from medical leave must be completed no later than one week prior to the start of the term.

Documentation must include a completed request for re-entry form, a clinician’s letter recommending the return to classes indicating: description of care given, diagnosis, medications (if applicable) and documentation of an appointment with a local provider, outside of UHCS, after re-entry. Students must attend classes on Northeastern campus for the term they wish to return from Medical Leave of Absence.

All documentation must be submitted on letterhead via fax, mail or email. The Care Coordinators will contact the student if there is missing documentation. Once all documentation is received by the MLOA Team, it will be reviewed and student will be notified of the decision.
